A) 26 Chestnut Street, Rye - NY:  Employees were exposed to fall hazards of approximately 19-feet while working on a ladder jack scaffold with no guardrail/end rail system or any fall protection devices, on or about November 14th, 2024.

29 CFR 1926.1053(b)(1):  Portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access:  A) 26 Chestnut Street, Rye - NY: Employees were exposed to fall hazards of approximately 18 1/2-feet while using an extension ladder to access a ladder jack scaffold. The extension ladder that was used, did not extend at least 3 feet above the scaffold landing, on or about, November 14th, 2024.
