29 CFR  1910.147(e)(3): Each lockout or tagout device was not removed from each energy isolating device by the employee who applied the device:   A) Grede DBA Grede Casting Integrity on or about September 27 at the address of 18771 Mill Street, Meadville, PA:  Maintenance removed Lockout devices from the energy isolating devices for Shaker #20.  The lockout devices were removed by employees who did not apply the devices. The procedures specified in the 29 CFR 1910.147(e)(3)(ii) through 29 CFR 1910.147(e)(3)(iii) were not followed when the Lockout devices were removed.
