29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about October 11, 2024, and at times prior, employees were exposed to finger amputation hazards during crimp press, terminal and wire connecting operations. The employer failed to ensure that there was a guard over the opening of the crimp presses located in the Wiring Mezzanine Building 2. An employee's right middle finger was amputated after reaching into the Mylar tape feed of the crimp press (Serial Number 1290 ; Model 638017600) while attempting to fix a jam.   b) On or about November 15, 2024, and at times prior, employees were exposed to finger amputation hazards during wire stripping operations. The employer failed to ensure the wire stripper's opening was guarded.
