29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.  (a) On the left side of the 221-223 building - On or about November 19, 2024, the employer exposed employees to struck-by hazards, in that, employees were not required to utilize protective helmets when working on the ground under framing activities above.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  (a) Building 221-223 - On or about November 19, 2024, the employer exposed employees to struck-by hazards, in that, the employer did not ensure that eye protective equipment was used when employees were exposed to eye injury while using pneumatic nail guns and circular saws.  (b) Building 217-219 - On or about November 19, 2024, the employer exposed employees to struck-by hazards, in that, the employer did not ensure that eye protective equipment was used when employees were exposed to eye injury while using pneumatic nail guns and circular saws.

(a) At building 221-223 - On or about November 19, 2024, the employer exposed employees to a fall hazard, in that, employees were not required to use fall protection while installing sheathing on the roof deck with a 9' fall to ground distance.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  (a) At building 221-223 - On or about November 19, 2024, the employer exposed employees to a fall hazard, in that, the side rails of the ladder being used for access to the attic area did not extend at least three feet above the upper landing surface.
