29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 11/20/2024 at 1230 Knollhaven Dr in Manchester, MO, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing. The ladder the employees used to access the roof was extended approximately .5 feet beyond the upper landing, or edge of the roof, thus exposing employees to fall hazards.
