29 CFR  1926.652(c): Designs of support systems shield systems, and other protective systems selected and constructed by the employer or his designee were not in accordance with the requirements of paragraph (c)(1); or in the alternative, paragraph (c)(2); or in the alternative, paragraph (c)(3); or in the alternative, paragraph (c)(4) as follows:   a) On November 20, 2024 and times prior, employees were exposed to cave-in hazards while working in a trench that measured approximately 7 ft long x 2.6 ft wide x 10.8 to 13-feet deep, with type B soil.   The vertical hydraulic shoring used did not fully comply with the tabulated data.    A single set of vertical shores was used at the bottom of the trench which was also undercut.  Two sets of vertical shores were placed staggered and next to each other near the top of the trench.  Note) Certification of corrective action is required for this item.
