29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices:  a)  Facility: On or about November 19, 2024, and at times prior thereto, the employer required employees to work on equipment without first being trained to ensure those employees had the knowledge and skills for the safe application, usage, and removal of energy control devices.

29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed:  (a) Room 126, on or about November 19, 2024, the employer allowed employees to work on or near deenergized circuits with exposed conductors (outlets without cover plates) without ensuring a lock and a tag were placed on the deenergizing means.
