29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.   On or about November 21, 2024, and at times prior thereto, an employee elevated masonry mortar via a pulley and rope system to the  2nd tier scaffolding platform for without protective helmets.

29 CFR  1926.502(b)(2)(i):Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level.   On or about November 21, 2024, and at times prior thereto, employees conducted masonry work from scaffolding greater than 10 feet above a lower level and midrails were not installed between the top edge of guardrail.
