29 CFR 1926.451(c)(1)(ii): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by installing guys, ties, and braces according to the manufacturer's requirements. a) Scaffold: On or about 11/22/2024, and at times prior, the employer exposed employees to collapse/fall hazards of 20 feet, 6 inches to 26 feet to the lower ground level while engaged in masonry activities, in that the tubular welded masonry walk-thru style scaffold system was not tied or braced to the structure to prevent tipping.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. a) Scaffold: On or about 11/22/2024, and at times prior, the employer exposed employees to fall hazards of approximately 20-feet, 6-inches and 26-feet to the lower ground level while engaged in masonry activities, in that a ladder was not provided to employees to access the scaffold. Employees climbed the masonry walk-thru type scaffold frames to access the working levels of the scaffold system.
