On or about November 25, 2024, and times prior to, employees were exposed to struck-by and crushing hazards when conducting warehouse tasks near pallet rack storage systems that were damaged due to forklift impacts. The storage racking systems had multiple damaged columns due to forklift impacts.  2. On or about November 25, 2024, and times prior to, the employer did not ensure that all rack columns were anchored to the floor with anchor bolts adequate to resist all applicable forces.
