Instances of failure include but are not limited to:    A.) Group lockout or tag out was not outlined within their program.  B.) Lockout or tagout device removal was not outlined within their program.  C.) Outside personnel (contractors) was not outlined within their program.  D.) Shift or personnel changes was not outlined within their program.  E.) Stored Energy was mentioned in the program but did not contain methods on how to verify, isolate, or remove residual energy.
