1926.652(c)(2)(ii)   Deviation from the specifications, recommendations, and limitations issued or made by the manufacturer shall only be allowed after the manufacturer issues specific written approval.      (a) On and before November 26, 2024, the employer did not ensure that employees were protected from injury from cave-in hazards, in that employees were working in an unprotected portion of the excavation that was approximately 16 ft in dept.  A portion of the excavation wall collapsed and pinned an employee.
