29 CFR 1910.176(b):  Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.  (a)  On or about November 26, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin; in Section 1 between Poles 6 and 8, boxes of material were not stacked in a stable manner due to the boxes leaning towards the aisle exposing employees to struck by hazards.  (b)  On or about November 26, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin; in Section 2 by Pole 24, pallets with boxes of material were not stacked in a stable manner due to the pallet was not supported with an approximately 6-inch overhang exposing employees to struck by hazards.  (c)  On or about November 26, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin; in Section 3 by Pole 48, pallet(s) with boxes of material were not stacked in a stable manner due to the boxes leaning towards the aisle exposing employees to struck by hazards.

Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace.   On or about November 26, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin, the employer failed to ensure that an employee who operated the Toyota propane forklifts were properly trained on the truck-related topics and work-related topics, evaluated and certified to perform the tasks in a safe manner, thereby exposing the employee to struck-by and caught-in-between hazards.

29 CFR 1910.178(p)(1):  If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.  (a)  During the week of November 11, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin; employees were exposed to hazards related to powered industrial vehicle operations while utilizing the Toyota propane forklift, Model:  6FGCU25, S/N:  65337, which was missing chunks of rubber on the right and left steering tires as well as the front right and left drive wheels.  (b)  On or about November 22, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin; employees were exposed to hazards related to powered industrial vehicle operations while utilizing the Toyota propane forklift, Model:  8FGCU25, S/N:  73208, which was missing chunks of rubber on the right and left steering tires.

Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected. (b)  On or about November 26, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin; in Section 3, the employer did not ensure that each fire extinguisher was mounted so that they were readily accessible to employees such as but not limited to a 5 lb multi-purpose dry chemical fire extinguisher located on the post east of Door 6, exposing employees to fire hazards.

Sufficient access and working space shall be provided and maintained about all electric equipment to permit ready and safe operation and maintenance of such equipment.  On or about November 26, 2024, at an establishment located at 920 North Bluemound Drive, Appleton, Wisconsin; in Section 1, the employer did not maintain at least 36 inches of clearance in front of the electrical breaker panel.  Access to working space and shutoff was obstructed by materials that were stored in front of the electrical box on the interior fire wall.
