29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder: a) Job site, 931 Woodward Avenue, McKees Rocks, PA: On or about November 27, 2024, during roofing work, the employer did not ensure that ladders used to access roofs at the site were properly extended in that the ladder used to get on/off the back porch roof was extended 18 inches or so above the roof line, and the ladder used to gain access to the main house roof was extended 24 inches or so above the roof line.

29 CFR 1926.20(b)(2): A program of frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons was not in use: a) Job site, 931 Woodward Avenue, McKees Rocks, PA: On or about November 27, 2024, during roofing work, the employer did not ensure that a system was in place to identify and correct safety and health hazard at the site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection, ladders not being extended above the landing areas properly, tools and equipment in contact with electrical lines, no head protection in use for ground level workers and ladders not being set up straight.
