OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that,  employees were exposed to struck-by and crushed-by hazards:  a) Throughout the site, located at 12790 U.S. 92 Plant City, FL 33566: On or about November 26, 2024, the Crown lift truck Model# C-51050-50 and Serial# 11094540 was operated without the use of the seatbelt, in that the seatbelt was not fastened while an employee was operating the forklift.   b) Throughout the site, located at 12790 U.S. 92 Plant City, FL 33566: On or about November 26, 2024, the Heli lift truck Model# CPCD25-KU14H and Serial# 010253A was operated without the use of the seatbelt, in that the seatbelt was fastened to the back of the seat while an employee was operating the forklift.
