29 CFR  1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye.  On or about November 19, 2024, an employee had a partial amputation of the right ring finger as a result of an at-work injury while grinding parts during their shift. The employer did not report the hospitalization to OSHA within 24 hours as required.
