29 CFR  1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    (a) 60-inch slitter: On December 16, 2025, the employer did not ensure that the energy control procedures that were established, were specific to the equipment such as the 60-inch slitter line and its components such as the rewinder and scrap winder, and clearly outlined the following: a specific statement of the intended use of the procedure (servicing, maintenance, and/or un-jamming), procedural steps for shutting down, isolating and securing the machine to control hazardous energy (electrical and hydraulic), procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them, and specific requirements for testing the machine to determine and verify the effectiveness of the lockout devices, tagout devices and other energy control measures.   No abatement certification or documentation is required for this item.

29 CFR  1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device prior to working on the machine or equipment:  (a) 60-inch slitter line: On or about December 16, 2024, the employer did not ensure when service and maintenance tasks were performed by a crew of more than one employee such as during the removal of a shaft on the 60-inch slitter line scrap winder that each employee affixed a personal lockout device.      (b) Garage Overhead Door: On February 1, 2025, the employer did not ensure when service and maintenance tasks were performed by more than one employee such as during the install of overhead heaters on the overhead garage door that each employee affixed a personal lockout device.    No abatement certification or documentation is required for this item.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) 60-Inch Slitter Line: On November 4, 2024, the employer failed to ensure adequate methods of guarding were used to protect employees where the location of the manual operating jog control allowed operator to access idle roll pinch point with other hand while operating the 60-inch slitter rewinder when taping down finished coils.     No abatement certification or documentation is required for this item.
