On or about November 29, 2024,  at the worksite, the employer provided 3M Half face-mask respirators with respirator cartridges, for voluntary use by employees while working with drywall,  cleaning molded rooms and using paint material, and had not developed and implemented the required elements of a  respiratory protection program.

On or about November 29, 2024,  at the  worksite in the hallways of the first floor, fire extinguisher that were hanging on the wall where locked and employees did not have any way to break the glass in case of fire, exposing employees to fire and entrapment hazards.
