29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  a) Job site, 18134 Beachwood Blvd, Pittsburgh, PA 15217: On or about December 6, 2024, six employees were installing solar panels on top of a residential roof at heights up to 15 feet above the ground and were not utilizing fall protection.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section.  a) Job site, 18134 Beachwood Blvd, Pittsburgh, PA 15217: On or about December 6, 2024, the employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill. Six employees were exposed to an approximately 15-foot fall hazard while installing solar panels on a residential roof.
