(a) On or about December 3, 2024, at a jobsite located at 6932 W Layton Ave Milwaukee, WI, the employer did not ensure that eye protective equipment was used when employees were using pneumatic nail guns, exposing employees to a struck-by hazard and eye injury.

(a) On or about December 3, 2024, at a jobsite located at 6932 W Layton Ave Milwaukee, WI the employer did not ensure employees working on a on 2:12 pitch residential home roof, and engaged in installation of shingles were protected from fall hazards. This exposed employees to a fall hazard of approximately 9 feet from the roof to the ground below.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  (a)On or about December 3, 2024, at the jobsite located at 6932 W Layton Ave., Franklin, WI 53220 the employer did not provide fall protection training to enable workers to recognize and control the fall hazards at their worksite while performing residential construction.  The workers were exposed to a fall hazard of up to approximately 9 feet.

(a)On or about December 3, 2024, at a jobsite located at 6932 W Layton Ave Milwaukee, WI, the ladder employees used to access the roof of the residential building, did not extend three feet above the landing surface, exposing the employee to a fall hazard of approximately 9 feet.
