29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.  a. On or about December 02, 2024, at 10105 Cedar Run, TAMPA, FL 33619, an employee was exposed to a fall hazard while removing and installing metal roof decking from a roof top with unprotected sides and edges more than 15 feet (4.6 m) above a lower level and were not protected by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761:   a. On or about December 02, 2024, at 10105 Cedar Run, TAMPA, FL 33619, employees were exposed to fall hazards while removing and installing metal decking from a roof top with an unprotected sides and edges more than 15 feet (4.6 m) above a lower level, during which the employer did not train each employee in fall hazard recognition, identification and prevention, and the use and operation of fall hazard prevention systems to be used, and the procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used, and the procedures to prevent falls through/into holes and openings.
