29 CFR 1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person. Location: Worksite at 258 Court Rd., Winthrop On or about November 5, 2024 employees were exposed to fall hazards of over 20 feet while performing roofing activities, where the employer did not ensure that a competent person made frequent and regular inspections of the site.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). Location: Worksite at 258 Court Rd., Winthrop a) Scraping the roof with fall hazards of over 20 feet. b) Standing and working on makeshift wooden platforms with fall hazards of over 7 feet in height at the front of the property. c) Walking, working, and exiting from unguarded 2nd floor window at back of property onto roof below, with fall hazards of over 12 feet. d) Unguarded 3- foot wide area of framing on the interior of the home with fall hazards of over 8 feet to basement below. e) Open floor holes at chimney demolition area on interior of home with fall hazards of over 8 feet to basement below. f) Open excavation area on the back left side of the home's exterior. On or about November 5, 2024 employees were exposed to multiple fall hazards while performing roofing and framing activities, where the employer did not ensure that fall protection was in place, by any means.
