29 CFR  1926.651(b)(4):While the excavation was opened, underground installations were not protected, supported or removed as necessary to safeguard employees:  a) E.R. Zeiler Excavation, Inc. On or about December 4, 2024, at the site location of 5820 Central Ave. Toledo, OH: The employer did not ensure a  natural gas line running perpendicular across an open excavation was adequately supported and protected against displacement.  Employees working in the excavation in proximity to the unsupported natural gas line were exposed to fire hazards.
