29 CFR  1926.100(a):Employee was  working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:   Worksite located at 740 NW Shoal Creek Parkway, Kansas City, Missouri- An employee was exposed to overhead hazards while working beneath another employees with tools and equipment overhead which could be dropped on him in which he was not wearing a hard hat.

29 CFR  1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Worksite located at 740 NW Shoal Creek Parkway, Kansas City, Missouri- Employee was exposed to possible eye injuries while the employee was observed applying stucco and had been mixing stucco without the use of eye protection.
