Gay Street, Warrensburg, Missouri- Employees were exposed to fall hazards while working on a steep roof and were not protected from falling by the use of any means of fall protection.

Gay Street, Warrensburg, Missouri- Employees were exposed to fall hazards while using a ladder that was not elevated at least 3 feet above the upper landing surface.
