29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) 140 Gulf Winds Ct. Destin FL. 32541; On December 5, 2024 and at times prior to, the employer permitted employees to work from a job-made work platform that was being elevated by a JCB Forklift Model# 509.42 , which was not approved for that use by the manufacturer, exposing employees to fall hazards of up to 25 feet.

29 CFR 1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l) and as referenced by the note contained in 1926.602(d): a) 140 Gulf Winds Ct. Destin FL. 32541; On or about December 5, 2024, and at times prior, the employer exposed employees to fall and struck by hazards in that the employer failed to ensure that employees were trained and certified to operate a telehandler forklift while employees were performing window installation.
