29 CFR  1926.501(b)(10):"Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a) On or about November 8, 2024, the employer failed to protect employees from falls while performing roofing work on a flat roof.  The employer did not implement a proper fall protection system, exposing employees to fall heights of 7'10.5" and 13'10.5"

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a) On or about November 8, 2024, the employer failed to ensure that the side rails of a portable extension ladder used by employees for re-roofing extended at least 3 feet above the upper landing surface. As a result employees were exposed to a fall hazard of 13'10.5".

29 CFR  1926.1053(b)(9): The area around the top and bottom of ladders shall be kept clear.  a) On or about November 8, 2024, the employer did not ensure the area at the base of the extension ladder was kept clear. Debris, including discarded materials and tools, obstructed the ladder's base. As a result, employees working on the northeast and southeast sides of the building were exposed to slip, trip, and fall hazards.

29 CFR  1926.1053(b)(22): Use. The following requirements apply to the use of all ladders, including job-made ladders, except as otherwise indicated: An employee shall not carry any object or load that could cause the employee to lose balance and fall.    a) On or about November 8, 2024, the employer failed to ensure employees did not carry materials while ascending or descending ladders. Employees were observed carrying a bucket of supplies in one hand while climbing, which prevented them from maintaining three points of contact on the ladder. This failure exposed employees to a fall hazard of 13'10.5" while accessing elevated work areas.
