29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   a) Worksite, downstairs packing department, on or about December 10, 2024 - An eye wash and shower station capable of providing 15-minutes of continuous flush was not available in an emergency situation when employees were exposed to splash and emersion hazards while cleaning parts inside the parts washer filled with Crystal Clean degreaser.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition:  a) Worksite, downstairs tube filler 1036 equipment,  on or about March 20, 2025 - Operators climbed and stood upon the top of a platform stepladder that was damaged, unlevel and cracked while stocking the inside of the tube filler.

29 CFR  1910.1200(h)(1): The employer did not provide employees with effective information on hazardous chemicals in their work area at the time of their initial assignment and whenever a new chemical hazard the employees have not previously been trained about was introduced into their work area. Employees were not informed of the following requirements of this section included in 1910.1200(h)(1)(i)- 1910.1200(h)(3)(iv):  a) Worksite, downstairs packing department, on or about December 10, 2024 - Employees were exposed to hazardous chemicals such as but not limited to: High Mod Clear R4421 (E6000), DC 15089 Silaprene adhesive which contained toluene and solvent naphtha, Crystal Clean a solvent degreaser which contained 100% petroleum, and IPA 99% which contained isopropyl alcohol during tube filler tasks. The employer did not provide information and training designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals; location and availability of the written hazard communication program and the relevant SDS sheets, nor the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used.
