29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about December 13, 2024, employees were exposed to struck by hazards, while performing roofing work on a residential home located at 751 Fairchild Ave, Kent Ohio 44240; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about December 13, 2024, employees were exposed to fall hazards, while performing roofing work on a residential home located at 751 Fairchild Ave, Kent Ohio 44240, the employer did not ensure employees working on a residential roof were protected from a fall of approximately 10.2 feet by utilizing a fall protection system.  Hilltop Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use fall protection), which was contained in OSHA inspection number 1691576, citation number 01, item number 01A and was affirmed as a final order on (12/8/2023), with respect to a workplace located at 1545 Lurchmont Ave, Lakewood Ohio 44107.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about December 13, 2024, employees were exposed to fall hazards, while performing roofing work on a residential home located at 751 Fairchild Ave, Kent Ohio 44240; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extended 3 feet above the upper landing surface.  Hilltop Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Ladders), which was contained in OSHA inspection number 1691576, citation number 01, item number 02A and was affirmed as a final order on (12/8/2023), with respect to a workplace located at 1545 Lurchmont Ave, Lakewood Ohio 44107.
