29 CFR 1910.147(c)(4)(i):  Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  At the worksite located at 1401 Junge Blvd, Joplin, Missouri:  Employees were exposed to caught in hazards in that the employer did not develop, implement, and ensure the use of lockout/tagout procedures.  a)  Employees did not lockout/tagout the machine prior to cleaning the die on the cracker stamper.   b)  Employees did not lockout/tagout the machine prior to removing and replacing the die on the cracker stamper.

29 CFR 1910.147(c)(7)(i):  The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  At the worksite located at 1401 Junge Blvd, Joplin, Missouri:  Employees were exposed to caught in hazards when working with the cracker stamper table.  The employer did not provide training on lockout/tagout to ensure the elements of the program were understood by employees whose work operations were in an area where lockout/tagout procedures are used.

Employees were exposed to the point of operation in that there was no guard around the die to prevent contact.
