29 CFR  1926.453(b)(2)(v):A harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a) Perram Electric, Inc. on or about November 13, 2024, near the intersection of SR 2 and Spring Meadows Dr. Holland, OH: The employer did not ensure employee(s) working from an articulating mobile platform donned and secured appropriate personal fall arrest systems. An employee installing a new traffic signal from the elevated platform without the benefit of fall protection was exposed to falls of 13'6" to the roadway below.

29 CFR  1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a) Perram Electric, Inc. on or about November 13, 2024, near the intersection of SR 2 and Spring Meadows Dr. Holland, OH: The employer did not ensure employee(s) operating an articulated mobile scaffold were adequatly trained to recognize and avoid hazards associated with operation of an aerial device.  An employee performing work from this device without the benefit of this training was exposed to falls 13'6" or greater to the roadway below.
