On or about 01/15/2025, at the site of 51-04 Douglaston Parkway, Douglaston, NY 11362. Employees installing new frame truss on a roof were exposed to the hazard of falling approximately 25 ft to the ground below. The employees were not protected by guardrail systems or personal fall arrest systems.

Chandrabali General Construction Expediting Service Inc. employee engaged in framing work on the second floor of a residential home was using the top step of an A-frame ladder and was exposed to a fall hazard of approximately 6 ft.
