29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Trademark Global, LLC doing business as, Trademark Games, LLC located at 7951 West Erie Avenue in Lorain, Ohio;  On or about December 18, 2024, the employer did not ensure the Alfamachine U500 Frame Joining Machinery was adequately guarded to protect operators from crushing hazards. Employees tasked with operating the Alfamachine U500 machinery to construct wooden frames were exposed to crushing hazards created between the wooden frame and the vertical clamp.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  Trademark Global, LLC doing business as, Trademark Games, LLC located at  7951 West Erie  Avenue in Lorain, Ohio  On or about December 18, 2024 the employer did not ensure that each chemical container storing hazardous chemicals such as, but not limited to, Clorox Bleach, Simple Green, Windex, and Uline 2 in 1 Disinfectant and cleaner, were labeled with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards. Employees using spray bottles of cleaning agents without adequate labeling on the mobile janitorial cart were exposed to hazards such as, but not limited to, eye, skin, and respiratory tract irritation.

29 CFR  1910.1200(e)(1)(i):The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet:  Trademark Global, LLC doing business as, Trademark Games, LLC located at  7951 West Erie  Avenue in Lorain, Ohio  On or about December 18, 2024 the employer did not ensure a list of the hazardous chemicals known to be present in the workplace was maintained as part of the company's written hazard communication program. A list of hazardous chemicals present in CLE#1 and CLE#2, such as but not limited to, ethyl alcohol, Clorox Bleach, Simple Green, and propane, was not established. Employees were exposed to hazards such as, but not limited to, eye, skin, and respiratory tract irritation.
