Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   15 Bay Meadow Road, Springfield, MA: On or about December 20, 2024, employees were observed working on a residential structure engaged in roofing activities with unprotected sides and edges greater than 6 feet above the next lower and were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall hazards.

The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   15 Bay Meadow Road, Springfield: On or about December 20, 2024, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained,  the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for employees who were performing work on a residential roof and exposed to fall hazards greater than 6 feet.
