An employee utilized an extension ladder on the front of the garage that did not reach the second level, exposing employees to a fall height of more than 14 feet.  b) Located at 2180 Overlook Way, Newark OH, 43055: On or about December 20, 2024, the employer did not ensure the ladder's side rails extended 3 feet above the landing for employees who accessed the top of the first level of the residential home while framing. Employees utilized an extension ladder on the east wall of the garage that did not reach the second level, exposing employees to a fall height of more than 13 feet.
