This condition exposed employees to struck-by and crushing hazards.   (b)Dr. Phil's Car Club LLC at 301 West 43rd Street, Sioux Falls, SD 57105: On or about and at times prior to December 19, 2024, the employer did not inspect the Nubbaum Hebetechnik Automotive Lift, Model SPL8000, s/n: 122060 for defects; exposing employees to struck-by and crushing hazards from falling loads in the event of lift failure. This condition exposed employees to struck-by and crushing hazards.  (c)Dr. Phil's Car Club LLC at 301 West 43rd Street, Sioux Falls, SD 57105: On or about and at times prior to December 19, 2024, the employer did not inspect the Nubbaum Hebetechnik Automotive Lift, Model SPL 8000, s/n: 122056 for defects; exposing employees to struck-by and crushing hazards from falling loads in the event of lift failure. This condition exposed employees to struck-by and crushing hazards. Abatement Note: Among others, the following are feasible and acceptable abatement methods to abate the hazard:   1) Institute a preventative maintenance program in accordance with ANSI/ALI ALOIM.  2) Ensure employees do not use automotive lifts with deficient safety devices.  3) Ensure unsafe lifts are effectively locked out until repairs can be made by qualified personnel.
