29 CFR 1926.1053(b)(1): Where portable ladders were used for access to upper landing surfaces, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladders were used to gain access.  At the jobsite located at 5736 Lowell Street, Overland Park, Kansas, employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 8 feet from the roof to the ground. The side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof or secured to prevent it from sliding. The side rails extended approximately 2 feet.
