29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  a) Production Area: On or about December 30, 2024, employees were exposed to struck-by hazards from operating an unguarded LVD Model #13340 Press Brake.   b) Production Area: On or about December 30, 2024, employees were exposed to struck-by hazards from operating an unguarded Cincinnati Serial #51922 Press Brake.  c) Production Area: On or about 12-30-2024, employees were exposed to caught-in and struck-by type hazards while operating the GL Fraiser machine serial #U49960 milling machines, with an inadequately guarded chuck portions near the point of operation.  d) Production Area: On or about 12-30-2024, employees were exposed to caught-in and struck-by type hazards while operating the TRAK DPM RX5 milling machines, with an inadequately guarded chuck portions near the point of operation.
