29 CFR 1926.416(a)(1): Employees were permitted to work in such proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:  (a) On or about January 4, 2025, the employer did not ensure that employees were protected from electrocution hazards while conducting snow removal operations on a residential roof, in the vicinity of energized 7.2 kV powerlines.  This resulted in an employee sustaining fatal injuries when he contacted the energized powerline.

29 CFR  1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take:   a) On or about January 4, 2025, the employer did not ensure employees were protected from electrical hazards in that, the employer did not determine if employees could contact the energized powerlines. The employer failed to post warning signs to advise employees of hazardous locations while conducting snow removal operations on a residential roof, in the vicinity of energized 7.2 kV powerlines. This resulted in an employee sustaining fatal injuries after contacting the energized powerline.
