29 CFR  1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazard.  On or about 30 December 2024, and at times prior there to, the employer did not ensure torch cutters wore leather flame resistant jackets while performing assigned duties.  This condition exposed employee to serious burn hazards.

29 CFR  1910.151(a): Medical personnel were not readily available for advice and consultation on matters of plant health:   On or about December 12, 2024 at at times prior thereto, the employer did not provide prompt access to health care for an employee that was burned.  This resulted in delay to medical access when the employee was transported to two different offices before receiving medical attention.
