29 CFR  1926.416(a)(3): The employer began work without determining if employees could contact any part of an energized circuit and did not post warning signs and advise employees of the location of the circuit, hazards, and protective measures they need to take:   a) On or about January 4, 2025 and at times prior, Knotty Iron LLC dba Story Renovations as the controlling employer, did not ensure subcontractor employees were protected from electrical hazards and did not determine if subcontractor employees performing snow removal activities could come in contact with energized 7.2 kV powerlines that were located over the roof surface of the residential building under construction. The controlling employer failed to post warning signs or advise subcontractor employees of hazardous locations while performing work on a residential roof, in close proximity of energized powerlines.

29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing hazardous conditions).   (a) On or about January 4, 2025 and at times prior, Knotty Iron LLC dba Story Renovations, as the controlling employer, did not ensure that subcontractor employees were protected from jobsite safety hazards by conducting frequent and regular safety inspections of the jobsite, materials and equipment by a competent person.  Subcontractor employees were exposed to electrocution and fall hazards while removing snow from the roof of a single-family home under construction.
