On or about January 8, 2025, at the above addressed jobsite, a wooden job-made ladder used to access the roof was not properly spaced 1' between rungs thereby exposing employees to the hazards associated with falls.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)   a. On Or about January 8, 2025, at the above addressed jobsite, the job-made ladder used to access the roofs surface did not extend at least three (3) feet above the upper landing surface, thereby exposing an employee to the hazards associated with falls.
