29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New residential construction located  on Lot #25/Block 1, Bundoran Dr., Evans, GA 30813, Roof: On or about 01/08/2025, the employer exposed an employee to struck-by hazard while operating a pneumatic nail gun, in that protective eyewear was not provided to an employee engaged in roofing activities operating these tools at 120 PSI.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) New Construction located on Lot #25 / Block 1, Bundoran Dr., Evans, GA 30813, Roof: On or about 01/08/2025, the employer exposed an employee to fall hazards of 12 feet, 8 inches to the lower ground level, in that a fall protection system was not provided to an employee engaged in roofing activities on a 6/12 pitched roof.
