29 CFR1910.178(m)(5)(i): When a powered industrial truck is left unattended wheels shall be blocked when parked on an incline.    Location:  406 South Orchard Street Wallingford, CT 06492  On or about January 10, 2025, the employer did not ensure an employee was provided with and required the use of a wheel chock while operating a forklift on an incline.  An unattended CAT GC40k s/n AT87B10449 forklift had rolled downhill striking the employee resulting in fatal injuries.
