On or about January 13, 2025, at 315 Melbourne Avenue, Merritt Island, Florida:  employees were exposed to 10 foot -9-inch fall hazards while conducting roofing activities on a 2:12 pitched roof, in that, fall protection was not used.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about January 13, 2025 , at 315 Melbourne Avenue, Merritt Island, Florida: employees were exposed to a 10 feet-9 inches fall hazard, in that a Multi-positioned ladder did not extend at least 3 feet above the upper landing surface.
