29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative On or about January 14, 2025, employees were exposed to caught-in and amputation hazards from performing servicing and maintenance activities, including but not limited to the Sanders, Saws, and the Glue Clamper, and the employer had not established a program consisting of energy control procedures, employee training, and inspections to ensure the startup or release of stored energy could not occur while servicing or maintenance.

On or about January 7, 2025, An employee operated a radial arm saw was exposed to amputation hazards because the saw did not have an effective guard. The guard in place was ineffective, exposing a portion of the blade.
