29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use.  On or about January 15, 2025, employees adding chemicals to the chemical reservoirs in the chemical/equipment room were exposed to corrosive materials to include but not limited to UF222 - Ultra Presoak 222, and UF260 - Body Shampoo 260; the employer did not ensure to provide a suitable facility for quick drenching or flushing of the eyes within the immediate work area.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure.  On or about January 15, 2025, a flexible cord was used in the chemical/equipment room as a substitute for fixed wiring to energize equipment up to 120-volts to include but not limited to the Accu-Pumps Box 1, Box 2, and Box 3.
