29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a) New residential construction located on Lot #2 at 3861 Lancaster Dr, Bethlehem, GA 30620, Front side of structure: On or about 01/15/2025, the employer exposed an employee to struck-by hazard while operating pneumatic nail guns, in that protective eyewear was not provided to an employee engaged in framing activities operating these tools at 120 PSI.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:  a) New residential construction located on Lot #2 at 3861 Lancaster Dr, Bethlehem, GA 30620, Roof: On or about 01/15/2025, the employer exposed  employees to fall hazards while working on a 10/12 pitched roof of approximately 19-feet, 10-inches to the lower ground level, in that a fall protection system was not provided while employees were engaged in framing activities.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step:  a) New residential construction located on Lot #2 at 3861 Lancaster Dr, Lot #2, Bethlehem, GA 30620, Front side of structure: On or about 01/15/2025, the employer exposed employees to falls of approximately 5-feet, 10-inches to the lower level, in that an employee worked from the top step and cap of an A-Frame ladder which is prohibited.
