Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net systems, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  A)  15 Virginia Place, Pleasantville - NY:  An employee was exposed to fall hazards of approximately 20-feet while engaged in roofing/sheathing work on the roof of a residential structure. The employee was not protected by any type of fall protection systems, on or about, December 6th, 2024.

The extension ladder that was used was only extended approximately 6 inches over the point of access; on or about, August 29th, 2024.
