29 CFR 1926.1053(b)(5)(i):Non-self-supporting ladders shall be used at an angle such that the horizontal distance from the top support to the foot of the ladder is approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support). a.) On or about October 29, 2024, the employer did not ensure that ladders used by employees for drywall installation work were set up at an angle equal to one-quarter of the working length(0.25); in that, the ladders were being used at a setup ratio of 0.52 (58.67 degrees), exceeding the required ratio of non-self-supporting ladders and exposing employees to fall hazards of 6'8" to a concrete surface below.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a.) On or about October 29, 2024, the employer failed to report an in-patient hospitalization of an employee who was injured in the workplace within the required 24-hour time period.
